In an analysis of the CRICO Strategies national Comparative Benchmarking System (CBS) of claims with anesthesia as the responsible service, researchers found that the most common contributing factors to the alleged adverse events were lack of technical skill, clinical judgment errors, communication mishaps, and documentation problems.
