This is a video walk through of how to obtain a physician claim history from XMAPS.

Welcome to the CRICO Underwriting Training Series. This video features information about how to obtain a claim history from XMAPS. This process will allow you to quickly obtain a claim history report for a physician with no claims. If the physician has a claim or if you are looking for a claim history for other employees such as nurse practitioners or physician assistants, please use the Claim History Wizard. For instructions on how to make a request with the wizard, please inquire with Underwriting or reference the Underwriting Training Series videos for more information.
We will now walk through the process of using XMAPS to download a claim history.

Log into XMAPS with your assigned username and password.

Click Underwriting underneath the Reporting tab.

Click 10Yr Claim History in the top left corner.

You can skip the Report Viewer dropdown box as it defaults to Adobe Acrobat PDF, which is the only option.

If you need to send the claim history to another individual who is authorized to receive the claim history, enter the email address into the Email Address field. Otherwise, you can leave this field blank.

Information entered into the Requestor Address field will appear at the top of the claim history report as the addressee. In most cases, you will want to enter the name of your organization. If you enter the physician’s name, include the title but make sure to use a comma to separate the name from the title. For example, Dr. Samuel M. Whittaker should be written Samuel M. Whittaker, MD.

Enter the physician’s last and first name into the Last Name and First Name fields then click Search.

Note that you may not need to enter the full name of the physician into XMAPS to locate the physician. If the name includes spaces or hyphens, you may need to try searching with different variations of the name. For example, if the first name is Mary Jane, you might need to try Maryjane or Mary-Jane.

Alternatively, you can search for the physician with the full Social Security Number if it is available.

A new dialogue box will appear. Choose the name of the physician for whome you are requesting a claim history report. If there are multiple physicians with the same name, use the Social Security Number to identify the correct physician.

If there are multiple physicians with the same name and the same Social Security Number, choose the one with a checkmark in the Primary column.

Once the physician you are requesting is highlighted in blue, click OK.

Use the View/Print button to either print or save the report to your computer as an Adobe PDF file.

If you are emailing the claim history, use the Email button to send the claim history to the email listed in the Email Address field.

Open the report to ensure that the organization or name entered in the Requestor Address field appears correctly on the report.

Note that, if the physician has a claim, the second page of the report will appear as it does here; claim details will not appear. To obtain a full report including the details of the claim or claims, please use the claim history wizard to submit a request.
