2016 AMC PSO: Quality, Risk and Patient Safety Leadership Day

Culture of Safety symposium and convening.

  • Member presentation: Led by Brigham and Women’s Faulkner Hospital and Brigham and Women’s Hospital, our presenters will highlight their journey to advance safety culture by developing a risk reliability operations model along with a “Just Culture” approach.

  • Workshop: Led by Paul LeSage of SG Collaborative Solutions, who will present a sustainable reliability model as a framework for improving risk mitigation and safety culture, including staff engagement and empowerment. This workshop, will also introduce the concept of using risk probability analysis for building effective action plans and how to integrate Just Culture through the adverse event reporting and investigation processes.

  • Convening: The AMC PSO will lead a facilitated discussion on high level leadership strategies and challenges to successfully implementing a Culture of Safety and developing a risk-reliable organization. This portion of the symposium, will be a protected and confidential convening discussion and will require confidentiality agreements and provisions as per the PSQIA regulations.
