2012 marks the 100-year anniversary of the Titanic tragedy, as well as 13 years since the IOM released the seminal patient safety study, “To Err is Human.” Both remind us that the realities of production pressure, technical skill, alarm fatigue, and failure to communicate continue to be at the root of our patient safety challenges.
