Identify the vulnerabilities that present risk to your patients and providers.
The Strategies RAP is an assessment that identifies the vulnerabilities that exist in a specific service, system, or an entire organization.
Each RAP assesses the ability of an organization to identify and respond to the vulnerabilities that put their patients and providers at risk. It explores how risk signals are captured, communicated, and used to drive system changes.
It is designed to provide institutions and/or their insurance vehicles (e.g., captives, trusts, commercial carriers) an understanding of both their strengths and vulnerabilities in the context of risk management and patient safety.
The RAP reviews the structure, maturity, capacity, and direction of the programs and processes that have a bearing on risk mitigation and patient safety with three specific goals:
The RAP assessment is based on the three dimensions of a model Patient Safety Profile: Structure, Culture, and Leadership and explores the key success factors related to a successful profile:
The RAP process, from initial document review to final presentation, is an engagement of approximately three months. Each phase relies heavily on the involvement and direction of key leaders and clinical providers/staff throughout the enterprise and all conclusions and recommendations draw upon the available data and feedback from all of the participants.
The RAP culminates in a workshop with key participants to discuss findings and develop a follow-up action plan. The intended outcome of this analysis is to ensure an organization is best positioned to identify, communicate and respond to risk-related vulnerabilities in a prospective manner, as well as activate appropriate responses, remediation efforts, and provider support in the face of an event.
