Letter of Intent: Purpose and Instructions

"Letter of Intent Cycle Closed as of 8/25/08" - Selected researchers will be contacted directly to submit RFP's.

CRICO/RMF is requesting a Letter of Intent as the first step in the 2008 Patient Safety Grant application process. Not to exceed three pages, the Letter of Intent should provide a brief summary of the intended project including objectives, approach, and predicted outcomes. It should include a focused hypothesis; a summary of methods and procedures; a description of the intended intervention (if any); and a clear discussion of potential outcomes.

The Letter of Intent should comprise one cover page and one summary page. It may also include one additional page with data and/or figures. It must be written in English and should be single spaced with 10 point font or larger.

I. The cover page must include the following:

  • Your Name
  • Title
  • Institution
  • Address
  • Telephone number
  • Email address
  • Proposal title
  • Identified Malpractice theme to be addressed (see overview)

II. The summary page should include the following:

  • a brief description of the problem to be addressed;
  • a statement of the project’s principal objectives, and who will benefit;
  • a description of the quality improvement strategy or education initiative;
  • the expected outcome and how it will be measured;
  • a plan for dissemination of findings; and
  • the estimated budget (budget cannot exceed $100,000). CRICO/RMF and HSRI have established a direct overhead expense rate of 10 percent. All funding awarded will include all direct and indirect costs.
