Referral Information

Recommendation: The provider communicates to the specialist complete referral information that includes the reason for the referral, basic medical information, and who will take ongoing responsibility for the patient.

 Exemplary Practice

  • A referral form is completed by the physician.
  • The appointment for the specialist is made by the office manager (while patient waits).
  • The LPN creates an envelope for the patient, including: time/date of the referral, a copy of the referral, and any special instructions.
  • The appointment is logged electronically. The LPN tracks appointments and makes remidner calls one week in advance of the appointments. Missed specialist appointments are rescheduled by the office.
  • Specialist reports are forwarded to the physician for review. They are dated and filed in a separate paper medical record with an additional note in the EMR.
  • The patient is notified of the consult report and new treatment plan recommendations.
  • The electronic list is used to reconcile and outstanding referrals and receipt of the specialist reports.
