PCP-Team Communication

Recommendation: The primary care provider is fully aware of the status and treatment plan for hospitalized patients.


 Effective Practice
The practice has a designated PCP (within the practice) who assumes a "hospitalist" role, and follows the group's inpatients
Benefits:
  • Improves access to MD attendance in the hospital
  • Enhances continuity of care
  • Promotes a hospital "expertise"
In practice at: Newton-Wellesley Internists (NWH), Southboro Medical Group (HVMA)


 Effective Practice
  • PCPs cover for their own hospitalized patients
  • When they are not available, this duty is assigned to the covering provider(s)
  • Communication may be oral, by phone, or e-mail
Benefit: Patient is seen by a physician familiar with his/her care
