Patient Notification of Test Results

Recommendation: A system is in place to notify all patients of test results and document the notification in the medical record.


 Effective Practice
Diagnostic test results may be conveyed to the patient by letter, phone, or through Patient Gateway. Providers receive an e-mailed receipt when patients view thier test results in Patient Gateway.
Benefits:
  • Patients are notified of both normal and abnormal results.
  • Patient gateway provides patient centered care and a mechanism to notify providers that the results have been reviewed by the patient.

 Effective Practice
  • Lab results are forwarded to the ordering provider's EPIC “In Basket” for review and acknowledgement.
  • Outside lab results are forwarded to the ordering provider, acknoledged, and scanned into the EMR.
  • Patient notification letters are generated from the EMR or results discussed at the visit.
Benefit: Patients notified of laboratory results automatically.

 Effective Practice
  • The provider reviews all test results.
  • The Medical Assistant or RN later mails a notification letter that includes provider instructions.
Benefit: Provider instructions are included on letter.

 Effective Practice
  • Patients complete test result cards in the office at the time of their blood draw to assure that results are received at the proper address
  • Patients are also given literature and told to call the office if they have questions about their lab results
