The administrator receives a centrally generated quarterly report of patients who have an abnormal Pap smear result. The list is given to the PCP and the MAs or nursing staff on the team perform the follow up with documentation in the medical record.
Benefit: Automated report identifies patients who might be a loss to follow up.

The practice receives a monthly “Abnormal Pap report” which is reconciled by the nursing staff with documentation of the follow up plan in the medical record.

 Effective Practice
Monthly lab report of Pap results. Medical assistants follow-up by phone with patients who have not scheduled an appointment. The patient will either be referred to gynecology or an appointment will be made for a repeat test at the time of the call.
