 Exemplary Practice

NSMC radiology generates reminders for abnormal mammogram follow up by using the VOCADA system to convey diagnostic findings to the PCP. The system generates a follow up message if the call is not answered.

 Effective Practice
The HVMA Radiology department generates a report sent to the PCP identifying patients with an abnormal mammogram and performs some follow up testing.

 Effective Practice
Abnormal mammogram results are called into the patient and PCP by Mount Auburn Hospital Radiology department. MACIPA sends a list of abnormal mammogram results every month that is reconciled by the medical assistants and PCP using the tickler file.
Benefits:
  • Institutionally generated list provides a back-up system for reconciliation.
  • Paper-based low tech easy to replicate.

 Effective Practice
The radiology department runs their own report for all abnormal mammogram testing results to ensure completion of recommended follow up. The radiology department will perform some immediate follow up on site. The results are sent to the ordering provider and often referred to a surgeon or the Breast Center. Urgent mammograms are scheduled by the referral coordinator who utilizes a monthly report for reconciliation.
Benefit: Promotes follow up on abnormal screening requiring follow up.

Paper system:
  • Mammograms requiring follow-up are placed in a logbook or computerized database
  • Staff person is assigned to review on a scheduled basis
  • Follow-up attempts are documented in the medical record

  • Paper-based / cost effective

 Effective Practice
Radiology notifies the PCP and the patient when follow-up mammogram is due following and abnormal finding. The PCP will contact the patient if the follow-up appointment has not been scheduled.

 Effective Practice
Mammograms performed at BIDMC are tracked by the QI administrative support staff to identify patients requiring follow- up. Patients requiring follow up are referred to the provider for further action.
